Student Employment - Employment Status Change Form
| To accurately process the student employment within your department, it is essential that this form be furnished to Human Resources prior to the second business day of each month. If you have many students in your area, you may complete the spreadsheet version. This form must be completed for all status changes to the student employee. This includes hiring a student or terminating a student. During the year, when a student receives a merit increase, the performance review will serve as documentation to increase the wage. Additionally, this is required when students return to their employment at the beginning of summer semester and academic year. Student will not receive paycheck unless this form has been completed (and student has completed I-9 and W4s.) |
