Student Complaint Tracking Form
Every college, school and office at Drake University is required to submit to the Vice Provost a list of complaints and appeals with which they have dealt each semester. If you have followed the "Complaint Process" in the College or unit where the concern originated a record of your complaint will be included among reports submitted to the Vice Provost.
If you have completed the formal complaint or appeal process in a college, school or other unit but wish to add more information for the record, you may complete this form. Please note that this does not constitute an appeal of any decision made through an official complaint or appeal process.
