Accident Reporting

Report all accidents involving Drake employees and student employees, no matter how minor, to the Human Resources and Safety offices using the First Report of Injury form located on the Human Resources web site.

If the accident results in an injury that requires medical assistance on site contact Security at 811 (campus phone) or (515)271-2222.

If the accident results in a life threatening injury call 9-911 (campus phone) or 911 (non-campus phone) and then call Security.

If the accident results in an injury that requires medical treatment but the person is able to go the clinic call Gary Johnson (271-4804) or Kris Rossmiller (271-3125) so arrangements can be made for the treatment of the employee.

Send the injured employee to a Concentra Medical Center. The locations of the Concentra are:

If a student is injured and it is not work related contact Donna Blunck at 271-3116.
