The goal of the screening and health risk assessment is to provide you with an increased awareness about your health. The health screening is not meant to take the place of your annual physical examination. The screening includes a blood lipid profile, resting blood pressure, height, weight, waist measurement, body composition, and a wellness consultation. By participating in the annual health screening and assessment, your medical contributions will be reduced.
Complete and submit a blue wellness enrollment form to Human Resources.
There are three screening options offered at various times throughout the year to accommodate your work schedule. You will receive detailed scheduling instructions, and with our on campus option you will also receive an appointment confirmation and reminder.
All individual information is kept confidential. The University only receives aggregate screening and assessment results which are used to develop wellness programs for faculty and staff.
