Proposal Routing Form

The Drake University Proposal Routing Form is used to help you and SPA keep track of your proposal and the necessary approvals. You are required to submit a Proposal Routing Form with each proposal. You should complete the following on the form before you bring your proposal to SPA; you can also refer to the Proposal Checklist.

Investigator/Proposal Data

  • Basic investigator data, including contact information
  • Basic proposal data, including project title, sponsoring agency, mailing deadline, project period and type of project
  • Amount of funds requested and the F & A cost rate used and Drake University cost sharing/match

Special Review Checklist

  • Acknowledgement of human subjects or animal research involvement
  • Space requirements, university housing or room and board commitments
  • Compensation for Drake faculty and staff; release time; need to hire new faculty and staff
  • Space requirements, university housing or room and board commitments
  • Compensation for Drake faculty and staff; need to hire new faculty and staff
  • Includes notification by Principal Investigator or Project Director and all co-investigators or co-directors of any potential financial conflict of interest (by checking "yes" or "no" in response to the conflict of interest question on the Routing Form)
  • Indication of subcontracts or consortia agreements
  • Description of project and relationship to the university mission

Investigator/Department Approvals

  • Please note: If any collaborators on the proposed project are in a different school or college than the Principal Investigator or Project Director, allow time to obtain signatures from all deans

Signatures of PI and all co-investigators

  • Certifying that they have read the statements of disclosures and assurances on the routing form and that the statements are accurate and truthful to the best of their knowledge

SPA will route your proposal to obtain institutional approval:

  • Signature of Deputy Provost
  • Signature of the Vice President, Finance and Administration
