Drake University

Request for Undergraduate Re-Enrollment

This form must be completed by undergradaute students who

a)  have been admitted to a Drake University degree program,

b)  have been enrolled in classes,

c) have not registered at Drake for at least one fall or spring semester, 

d)  and wish to re-enroll at Drake University. 

Applicant:  Please read this form carefully, print it, complete it and submit to the Dean's office of the college or school in which you were last enrolled. 

IMPORTANT STUDENT INFORMATION

Student must contact the Dean's Office of their last enrollment.

Student is responsible for furnishing all official transcripts from schools attended since leaving Drake, if any. Registration will be held up for subsequent semesters if the documents are not submitted on a timely basis.

Send ALL transcripts to the Office of Student Records, 2507 University Ave, Des Moines, IA 50311.

Student is responsible for clearing any hold before re-enroll approval or registration.

Any student seeking financial assistance should contact the Office of Student Financial Planning.

Student must contact the Housing Office if on campus housing is desired.

Any international student must contact the International Center to obtain, complete and return a new Financial Certificate as soon as possible.

Student is responsible for any college/school specific re-enrollment requirement.

Student will be notified of enrollment status decision.
