New Student Forms

Even if you’ve previously been seen at the counseling center, new forms must be completed before your first appointment each year to ensure we have the most up to date information. Please only complete the form before your first appointment during the current school year. 

Please complete these forms only after you’d made an appointment. Completing these forms do not alert the clinic you’d like to make an appointment.

As a reminder, you must be in Iowa at the time of your appointment to be seen at the counseling center. If you are not in the state at the time of your appointment, your counselor will ask you to reschedule.
