Request for Undergraduate Re-Enrollment

This form must be completed by undergraduate students who

1. have been admitted to a Drake University degree program,
2. have been enrolled in classes,
3. have not registered at Drake for at least one fall or spring semester,
4. and wish to re-enroll at Drake University.

Applicant: Please read the Re-enrollment Form re-enrollment form carefully, print it, complete it, and submit to the Dean's office of the college or school in which you were last enrolled.
