Accommodation Requests
Disclosure and Request for Disability Accommodations: This form is for all new students requesting services through the Access and Success Office. After submitting the form, students should schedule an appointment through Starfish to discuss reasonable accommodations and implement approved accommodations.
Disability Documentation May Include:
- Academic accommodations: A letter from a licensed medical, mental health, or other qualified provider; an evaluation of your learning, psychological, and/or cognitive profile; or an evaluation/reevaluation from your high school (e.g., IEP, 504 plan, etc.)
- Housing accommodations: A completed Housing Accommodation Provider Form from your licensed medical or mental health provider
- Emotional Support Animals (ESA): A letter on official letterhead from a licensed medical or mental health provider that supports your need for an ESA in housing, the animal's most recent vaccination records, and a clear photo of the animal
- Dining accommodations: A completed Dining Accommodations Provider Form or a detailed letter on official letterhead from an appropriate licensed provider
- Temporary accommodations: A statement from your provider outlining the need for a temporary accommodation. If requesting temporary accessible parking, documentation must include the anticipated duration for which accessible parking is needed
