Accommodation Requests

Accommodation Request Form: This form is for all NEW students to Student Disability Services in the Access and Success Office. After completing this form, the final step is to schedule an appointment in Starfish to discuss reasonable accommodations and put accommodations in place. 

Accommodation Change Request Form: This form is for all RETURNING students to Student Disability Services in the Access and Success Office that have received Accommodations at Drake and would like to modify or CHANGE their current accommodations. Depending on the change, an Access and Success staff member may be in touch to schedule a meeting to discuss more. You may also schedule a meeting in Starfish here if you would like with Michelle Laughlin.
