 Request to change Major/Minor/Graduation Date

3. From the Student Services card, click Change of Record & complete the form

The below forms can be accessed via the Office of the Registrar:

• Add/Drop Form (editable PDF)
• Transcript Request Form
• Transfer Credit Request Form
• Re-enrollment Request Form
• Medical Leave of Absence Form
• Registration Waiver Request Form
• Credit Overload Request Form

For the changes noted below, please use this form: Registration Waiver Request Form

• Students may request waiver of certain pre-requisites or course restrictions. 
• Requests are subject to review by the course's instructor or the Dean's office of the College or School offering the course.
• Students are responsible for registering for the course in Self-Service if a waiver is approved.
