If you or someone you know has experienced sexual or interpersonal misconduct, you may inform the University by submitting an online incident report to the Title IX Coordinator.

Sexual and Interpersonal Misconduct Incident Report

Although you may report anonymously, we encourage you to provide your name and contact information so we can provide you with information about additional resources and processes available to you. We will follow up to the extent possible with the information you have provided and any additional information you choose to provide in the future. Our ability to take remedial action may be limited should you choose to report anonymously.

 

Anyone may submit an incident report regarding behavior prohibited by the Sexual Harassment Policy, including sexual assault, sexual harassment, sexual exploitation, domestic violence, dating violence, stalking, or retaliation. Submitting an incident report does not require you to participate in a University complaint resolution process, nor does it require you to contact law enforcement.

Emergencies

In the event of an emergency, call Drake Public Safety at 811 or Des Moines Police at 911.


Faculty and Staff

In accordance with the University's Sexual Harassment Policy, all University employees (except Student Counseling and Student Health employees) must report information they have about possible sexual or interpersonal misconduct to the Title IX Coordinator.

*Employee reporters submitting reports through this form should follow up with the Title IX Coordinator at 515-271-4956.

After your report has been submitted

The Title IX Coordinator will evaluate the information received and determine what further actions should be taken. The Title IX Coordinator will take steps, either directly with the complainant or through the person making the report, to provide information about reporting options and as well as health and advocacy resources.
